ATLANTA – Insurance and Safety Fire Commissioner John F. King announced today that warrants have been issued for the arrest of Melissa Glass, 46, for one count of Insurance Fraud and one count of Forgery in the 1st Degree. Ms. Glass is wanted in connection with a Polk County case.

“Our Criminal Investigations Division discovered that Ms. Glass initiated an accident policy claim and a disability policy claim for a total amount of $1400,” said Commissioner King. “The accused falsely claimed she was hospitalized and disabled for a period of time. Ms. Glass then provided forged medical documents to Colonial Life and Accident Insurance to substantiate her insurance claim.”

On May 18, 2021, arrest warrants were issued by a Polk County Judge.
