Fire Fatality Report

  • Who reports

    All Fire Fatalities - civilian or firefighter - must be reported to the Georgia Office of Commissioner of Insurance and Safety Fire.

  • Timeline & Method
    • Within 72 hours of the incident, submit a report via the State Required Method
    • UPDATE (CHANGE TO REPORTING) as of 3/15/2026 Form 
      • All Fire Fatalities must be reported utilizing the Fire Fatality Data Collection Hub provided by the National Fire Marshal's Association. 
        • The goal of this form is to provide states with a repository for information and analysis of fatal fires. 

    Legal Basis - Under Georgia Code Title 25, Chapter 2, Section 25-2-32, all fire incidents - including those resulting in fatalities must be formally reported to the State Fire Marshal's Office using approved methods.
