REGISTRATION INFORMATION

  • Complete the registration request for the GIMS system. Information is provided at the top of this page
    • Be prepared to provide all contact information, including business and mailing address and phone numbers
    • Complete the citizenship affidavit and ID for the responsible party
    • After approval for registration account in GIMS, log into system and apply
      • Provide all information requested
      • Upload the citizenship affidavit and ID
      • Pay the $50 filing fee and $5 processing fee

RENEWAL INFORMATION

  • Log into the GIMS system
  • Be prepared to provide any updated contact information, including business and mailing address and phone numbers
  • A citizenship affidavit and Id are only required on renewals if the responsible party has changed or if the responsible party is a non-citizen. Upload if required.
  • Pay the $50 filing fee and $5 processing fee

The only form needed for this registration is the Download this pdf file. /document/document/citizenship-affidavit-and-instructions-gid-276-en/download . The previous 240-NT form is no longer used. The filer will enter the contact information needed for this registration when applying. At renewals, the filer will verify and update any contact information in the record.
